No. Dy.GMF(Funds)/GLI-2024-27/ 3.6.0.2 - 69 Date: 24 - 07 - 2024 OFFICE MEMORANDUM SUB- GROUP LIFE INSURANCE CONTRACT FOR EMPLOYEES OF WAPDA, DISCOS, GENCOS ETC., & REVISED SCHEDULE FOR GLI DEDUCTION & SUM ASSURED W.E.F 01.07.2024 TO 30.06.2027. Ref: Consent Letters of DISCOS/GENCOs etc. on above subject in response to our letter dated 356-71 dt-14.02.2024. 1. With regard to consents referred above, it is applied that Competent Authority has approved following three contracts for further 3 years w.e.f. 01.07.2024 to 30.06.2027. a. General GLI Contract for all employees with State Life Insurance Corporation: DDO of each office must deduct GLI Contribution (i.e. Premium from salaries of all regular contractual and lump sum package employees (to whom the GLI facility is given in their appointment letter) on monthly basis as per revised schedule given in part 2 below and submit those deductions to this office up to 10th of next month. b. ADBR Contract for LAH & LIH II with State Life Insurance Corporation: Each Company has to provide data of LAH & LIH II working under its jurisdiction at the end of each financial year. On the basis of that data, this office will issue annual premium invoices for ADBR Premium to CEO/MDCFO of each Company. In case of delay in provision of current data, this office will issue provisional invoice on the basis of latest available data, which will be adjusted later on upon provision of actual employee data for relevant financial year. Respective Company is bound to pay each ADBR invoice within 30 days from date of issue otherwise ADBR cases of that Company will not be entertained. c. ADBR Contract for ALM with EPU Insurance Limited: Each Company will provide data of Certified ALM working under its control. Consequently, this office will issue annual premium invoices for ADBR Premium to CEO/MDCFO of each Company. In case of delay in provision of current data, this office will issue provisional invoice on the basis of latest available data, which will be adjusted later on upon provision of actual employee data for relevant financial year. Respective Company is bound to pay each ADBR invoice within 30 days from date of issue otherwise ADBR cases of that Company will not be entertained. 2. Revised monthly premium rates of General GLI deductions for each Basic Pay Scale groups are indicated below (w.e.f. 01.07.2024). Sr. # BASIC PAY SCALE NO. SUM ASSURED (Rs.) However, as sum assured of all employee’s categories with relevant BPS has increased in this contract, so deductions have also increased attributed with relevant sum assured. 3. Drawing & Disbursing Officers are requested to send General GLI Deductions as per revised schedule along with respective GLI deduction schedules. Demand Drafts without GLI schedules and old rates will not be accepted/entertained. 4. As per terms of new Contract (01.07.2024 to 30.06.2027) maximum time limit for lodging GLI claim/ADBR claim is 59 months from date of death. Afterward claim will become time barred hence insurer will not pay the claim. Consequently, concerned formation/DDO will be responsible for no-payment of GLI/ADBR claims. 5. Method of submission of GLI claims All GLI claims of deceased employees must reach in this office within 31 days from the date of death. If any dispute exists among the legal heirs then GLI claim must be sent to this office without succession certificate/GLI Nomination Form. GLI claim must be accompanied the following mentioned documents with covering letter bearing contact numbers, complete address of Drawing & Disbursing Officer. Two Original Claim Performa duly signed by DDO as per specimen attached. Two attested copies of death certificate issued by NADRA/Union Council. Two attested copies of CDC showing the GLI deduction date as per BPS Group. Two attested copies of CNIC of deceased employee and nominated beneficiary each. Original GLI nomination form or attested copy of GLI nomination form duly Countersigned by DDO with official stamp, Assistant Managers (Operation) are not DDOs. Attested copy of 1st and last page of service book. Annual GLI list page showing the name of deceased employee. Two attested copies of the Full Salary at death showing GLI deduction as per BPS. 6. Method of submitting ADBR Claim ADBR claim is admissible for LM-I, LM-II & certified ALM who died due to electric shock, falling from pole, pole fallen upon him, burning from transformer/oil while performing official duties. ADBR claim must be accompanied by above mentioned documents (para 5) along with electric shock certificate (as per specimen attached) & inquiry report issued by Dy. Manager (safety). In case of certified ALM T-100 completion course certificate may also be provided & name of the deceased ALM must be included in annual list of certified ALM’s of previous year provided by Director/DC HR Admin of concerned company. 7. Employees Data As per terms and conditions of the contract the calculation of Annual Premium is made on the actual strength of insured employees. It is therefore requested the data of insured employees in the following summaries /list as on 30th June of every year in consolidated form may be provided at the earliest as under: Working Basic pay status summary of insured employees (As per Annex-A) List of insured employees’ data as per (Annex-B) Regular contractual and Lump Sum packages employees whom GLI deduction made from their salaries on monthly basis. Basic Pay scale wise summary of all LM-I, LM-II (As per Annex-C) A List of qualified ALM fulfilling the mentioned criterion (As per Annex-D) Please provide requisite data on top priority basis, (Hard copy through courier and soft copy at wapdagli@gmail.com) 8. Ensure the completion/revision of GLI nomination Form of all employees as per family definition of GP Fund Rules and the same must be placed in their respective Service Books & Personal Files to avoid Succession Certificate from Court of Law. 9. All Chief Executive Officers of the Companies/Formations and Head of Divisions are requested to direct/inform lower offices under their administrative control to ensure strict compliance of all the above instructions (particularly para 4 above).
